Community Health Teaching Project
This assignment will be submitted in 2 parts. The student will outline a Community Health Plan and Health Education Lesson Plan for his/her community and implement this in his/her community.  Students will use the following steps to formulate a community health nursing project.
Part I – Submit a two-page proposal that follows APA reference format and includes the following:
Assessment: Assess a targeted population in your community to see what needs to exist. This is done by researching health data for your population. Make sure you select and define your community by noting the history of your community.
Capture a picture of your selected community. What are the vital statistics of your community?  Think about socioeconomic statistics. What are the leading causes of death? What illnesses or diseases are present in your community? Do citizens in your community have easy access to primary care? What are the most prevalent health problems in your community?
Formulate a community health nursing diagnosis related to this health need, based on this data, what are the strengths and needs of your community?
Plan community intervention. Ask yourself if this will have a positive health impact in the designated population? How do you know?
Implementation. Create an educational presentation that will address the most important needs of your population. The presentation must be arranged with your instructor. The instructor will attend your presentation virtually or in person.  (PART II of your Teaching Project)
Evaluation: How would you evaluate your intervention

Assessment included in Proposal:
It has been identified by the Development Director at Residential Plaza Blue Lagoon need for staff training regarding N-95 mask use. Targetted audience/Community of interest:  Staff, such as Nursing assistants, Home health Aids and Patient Care Coordinators working at an established ALF facility with over 100 elderly residents.
Students will perform a Literature Review of the correct use of N-95
Students will meet to determine how the presentation will be provided to staff, via Zoom?,  Video-conference?, Video taped?, English and Spanish?
Students will develop goals and objectives for the presentation and an Evaluation tool which will be emailed along to the responsible party at the agency.
Implementation:
Students will have 2 weeks to have the presentation prepared and ready for professor approval.
Students can choose to do presentations to staff at two or three different times during the next 2 weeks, via indirect video-conferencing or send the video presentation to the Development Director and collect the Evaluation tool at the end of 2 weeks. Follow up required with Development Director via email as to how is the presentation being perceived by the staff, how many times was the presentation given and to how many participants.
Evaluation:
Students will receive the Evaluation tool response via email and will analyze data.
Short report will be written as to what the evaluation demonstrated and be sent to the professor.
